Booking an appointment
You will require a referral from either your GP or specialist.

Your GP referral will last a period of 12 months from the date of your first appointment unless your GP provides you with an indefinite referral.
Staff will be in contact with you for further information upon receipt of this referral.
Alternatively, you can contact the clinic with your information and staff will assist you with your booking request.

As soon as you know the date of your appointment
You’ll need to have a copy of your referral from your doctor. Please bring this referral to your first appointment along with any scan reports from previous imaging you have completed in relation to your referral including nerve conduction studies/xray/MRI/CT/bone scan/BMD etc.

Completing a Patient Information Form
This is available to you on this website and can be printed and completed prior to your appointment. Please remember to bring this with you on the day of your appointment.
You are welcome to scan this and email it to our clinic prior: to info@seaviewclinic.com.au and this will be saved to your file. You can also bring your completed form to your appointment if you don’t have access to a scanner. If you’ve been sent a PDF copy of the form, you can complete it electronically and email it back.

Returning for a follow-up appointment?
Please complete all referred scans prior to your follow-up appointment. If you have any questions please contact staff.
Tell your support people
If you have a partner, relative or friend whom you would like to be fully informed about your condition, they’re welcome to accompany you to your appointment.
You may also find it helpful to bring a written list of questions to ask.
3 days before your appointment
You will receive a courtesy SMS reminder regarding your appointment. Please reply ‘Y’ to confirm your appointment.
Can I no longer attend? Please reply ‘N’ or contact the clinic to make alternative arrangements. Staff will be happy to help you.
The day before your appointment
Please check that you have these items arranged:
If possible, the Patient Information Form is completed and emailed back to us or ready to bring.
All past and recent scans and reports
A list of your current medications (if not included in your referral)
Medicare card, Pension card, or Veterans Affairs card
Your private health fund details, if you have private health insurance
Your pre-approval letter, if Worker’s Compensation applies to you.
On the day of your appointment
Fees
Fees are payable at the time of consultation. Payment on the day can be made using cash, credit card (Visa or Mastercard) or EFTPOS.
Our staff will offer to do your Medicare claim for you on the day.

Non-attendance or cancellation
If you fail to attend or cancel with less than two business days’ notice, a cancellation fee will apply. As appointments are booked months in advance, please let us know if you need to reschedule so your appointment can be offered to someone else.
Emergencies
From time to time Dr Cherukuri is required to attend to emergencies as we are doing our best to look after all our patients.
If this occurs on the day of your appointment we will contact you as soon as possible and provide you with as much notice as possible.
You are more than welcome to contact the clinic on the day of your appointment and see how the doctor is running for time. This may help with your waiting time if you are unwell or uncomfortable sitting for too long.
If you are unwell on arrival, please advise the staff immediately.
Public (Medicare only) Patients
You may be referred for scans/investigations prior to your clinic appointment with Dr Cherukuri. These are vital diagnostic tools incorporated in your clinical assessment and treatment plan. Your scans need to be completed not more than 6 weeks prior to your appointment.
If you are completing imaging outside of the Illawarra or Shoalhaven District, please notify staff so that we can best prepare for your appointment.
Worker’s Compensation
If Workcover is involved in your injury/condition, you will need to arrange an approval from your insurance company/case manager stating that you are pre-approved for your consultation. This will need to be received prior to your appointment and can be emailed to the clinic.
No approval received? We will be asking for you to pay upfront and then you can claim back from your insurer.
Third-Party/Accident Injury Compensation Claims
Dr Cherukuri does not see third-party claims, either with or without approval. If you would like to see him as a private patient for an injury then you will be classified as a public or private patient.
